Once you have indicated all the post-commitment changes that you would like to make, you will be routed to a Preview menu showing all form entries and requested changes.

• Please carefully review all three tabs: Request Details, Contact Information, and Requested Changes.
• Once you have finished reviewing, click Continue to Certification.
